What to Expect at Your First Vascular Examination

If you’ve been referred for a vascular exam, you might be feeling uncertain about what comes next. Perhaps your primary care doctor noticed something unusual, or you’re experiencing symptoms like leg pain, foot wounds that won’t heal, or swelling that doesn’t go away. Maybe your physician suspects you have peripheral artery disease (PAD).  No matter the reason, this first step could be one of the most important for your health and mobility.

Why You Might Be Referred to a Vascular Specialist

Not every vascular issue presents with dramatic symptoms. In fact, many people live with limited blood flow for months or even years without realizing the root of their discomfort. The most common reasons for referral include:

  • Slow-healing or non-healing foot wounds

Getting to Know You: The Initial Conversation

Your first visit will begin with a detailed conversation. This is your opportunity to share what you’ve been feeling, how long symptoms have been present, and how your health has changed over time. The provider will review your full medical history, including any surgeries, ongoing conditions like diabetes or hypertension, and a list of medications you’re currently taking. A Hands-On Vascular Evaluation

After the consultation, your provider will perform a focused physical examination. This includes evaluating the strength of the pulses in your feet and legs. At Southern VIP, we use a TBI (toe branchial index), the gold standard for checking relative blood flow to the feet. We also check for temperature differences, inspecting the skin for ulcers or discoloration, and observing any visible signs of swelling or vein damage.

Most Southern VIP patients will also receive a vascular ultrasound at their first appointment. This quick and painless test uses sound waves to visualize the blood flow through arteries and veins. At Southern VIP, our technicians use advanced ultrasound technology to detect narrowed arteries, blocked vessels, and signs of clot formation in the legs and feet.

Understanding Your Treatment Options

Once your exam and imaging studies are complete, your provider will explain the findings in clear, understandable terms. You’ll learn if your symptoms are related to narrowed arteries, faulty vein valves, or another vascular condition like Deep Vein Thrombosis (DVT). If your condition doesn’t require immediate intervention, we’ll work with you on a plan for lifestyle changes, medications, and regular monitoring.

In cases where circulation is compromised, the provider will review treatment options, which may include minimally invasive procedures.  Southern VIP’s team specializes in these outpatient procedures, including angioplasty, stenting, and our signature Foot Rescue™ procedure, which treats reduced blood flow to the lower legs and feet, all the way to the toes. These techniques require no surgical cutting but help restore healthy circulation, relieve symptoms, and, most importantly, reduce the risk of serious outcomes like limb loss.

If you are experiencing any wounds on your feet or lower legs, the provider will closely examine these areas to determine how blood flow, or lack of it, might be impacting healing. The physical exam is thorough but gentle, designed to gather valuable insights while keeping you comfortable.

If more information is needed, additional imaging, such as CT angiography or MR angiography may be recommended. These tests provide more detailed views of your vascular system and are typically reserved for patients with advanced or complex issues.

Preparing for Your Appointment

Patients often ask how to prepare for a vascular exam. While the process is simple, being organized can make the experience smoother.
Wear loose-fitting clothing that allows access to your legs and arms. Bring a list of your medications, prior diagnoses, and recent lab results or imaging, if available. It’s also helpful to write down any questions you have beforehand. Also, know your numbers (i.e., A1c, LDL cholesterol), and bring along any testing that has been done in the last 12 months.
Some patients choose to bring a loved one along for support or to help take notes. Your visit is a partnership, and at Southern VIP, we encourage open communication every step of the way.

Not necessarily. Many vascular conditions can be managed through medication and lifestyle changes. At Southern VIP, procedures are only recommended in more advanced cases. Our minimally invasive procedures differ from surgery in that they do not involve cutting or a hospital visit and allow the patient to go home the same day.
Be sure to mention any resting leg pain, nocturnal leg cramps, numbness, tingling, swelling, wounds that won’t heal, or temperature differences between limbs. Even symptoms that seem minor could point to circulation issues.
The entire visit—including the consultation, exam, and basic ultrasound—usually takes between 45 minutes to an hour. If advanced imaging is needed, a follow-up appointment may be scheduled.
